1 Introduction In patients undergoing surgery for head and neck cancer, wide local excision can result in a large composite tissue defect. Head and neck reconstruction often relies on vascularized free tissue transfer to achieve functional and aesthetic outcomes following wide local excision [1]. Flap failure is one of the most dreaded complications in free flap surgery, and great care is taken in the perioperative timeframe to closely monitor flap viability.

Traumatic expanding intrathyroidal hematoma is rare and can be clinically obscured, leading to delayed diagnosis and potential airway compromise. Identification and prompt intervention are paramount to survival. Patient presented to the emergency department as a modified trauma without signs of respiratory distress or enlarging neck circumference. Comprehensive trauma imaging identified the thyroid pathology prompting otolaryngology involvement.
