Three months after WHO declared the Bundibugyo Ebola outbreak in the Democratic Republic of the Congo (DRC) and Uganda a Public Health Emergency of International Concern (PHEIC), the epidemic has taken sharply different trajectories in the two countries. Uganda declared its outbreak over on 28 July after 42 days without a new locally transmitted case. In the DRC, however, transmission has continued to accelerate.

For decades, development assistance has defined the relationship between the United States and the African continent.

To move from debate to delivery, global health architecture reform must rebalance power, reinforce systems and restore trust, with a sharper focus on managing geopolitics and emerging threats. The global health architecture is under acute strain. Pandemic threats, climate change, armed conflict, economic inequality and geopolitical tensions are intersecting at a moment of eroding multilateralism. This generates compounding risks that respect neither borders nor sectors.
