Abstract Background: Alterations of acid–base balance induced by intravenous fluids are primarily related to changes in plasma strong ion difference (SIDPL) and nonvolatile weak acids. Previous studies suggest that the relationship between the SID of infused fluids (SIDINF) and pre-infusion plasma bicarbonate (HCO3−) may act as an integrative factor influencing acid–base changes during fluid replacement.

Introduction: Metformin intoxication causes lactic acidosis by inhibiting Krebs’ cycle and oxidative phosphorylation. Continuous renal replacement therapy (CRRT) is recommended for metformin removal in critically ill patients. According to current guidelines, regional citrate anticoagulation (RCA) is the first-line strategy. However, since metformin also inhibits citrate metabolism, a risk of citrate accumulation could be hypothesized.
