Abstract Background Cancer incidence increases with age, so some clinical guidelines include patient age as one of the criteria used to decide whether a patient should be referred through the urgent suspected cancer (USC) pathway. Little is known about how strictly GPs adhere to these age criteria and what factors might influence their referral decisions for younger patients.

The incidence of some malignancies, such as colorectal cancer, in younger patients is steadily increasing worldwide,1–3 with a 2023 study indicating a 28% rise in mortality due to early-onset cancer in 2019 compared with 1990.3 However, most types of cancers remain uncommon in people aged <50 years.
