In Reply We thank Dr Morelli and colleagues for their thoughtful comments about the OPTION trial.1 Regarding the concern that mild baseline neurological deficits might influence favorable outcomes (mRS score range, 0-1), randomization successfully balanced patients with NIHSS scores of less than 6 between the 2 groups (90 with tenecteplase vs 92 controls).
