Vagus nerve stimulation effects are investigated in epilepsy patients with developmental and epileptic encephalopathies (DEE) or without intellectual disability (ID). Seizure reduction increased over time for all patients, even with unchanged antiseizure medications. Atonic seizures had the highest median reduction in seizure frequency. Most adverse effects were mild and improved over time; profiles and frequencies of reported adverse events differed between patient groups.

Abstract Contextual cues and prior evidence guide human goal-directed behavior. The neurophysiological mechanisms that implement contextual priors to guide subsequent actions in the human brain remain unclear. Using intracranial electroencephalography (iEEG), we demonstrate that increasing uncertainty introduces a shift from a purely oscillatory to a mixed processing regime with an additional ramping component.
