Tumor-associated Schwann cell remodeling under metabolic stress via lactate sensing orchestrates pancreatic ductal adenocarcinoma development
Introduction Pancreatic ductal adenocarcinoma (PDAC) presents significant challenges due to its elusive onset, rapid progression, and inherent resistance to therapeutic interventions.1,2,3,4 Metabolic alterations driven by PDAC, along with changes in the pancreatic microenvironment, can lead to new-onset diabetes.5 In addition, obesity and type 2 diabetes mellitus (T2DM) are significant risk factors for PDAC,6,7,8 often detected before the cancer diagnosis.9,10,11,12,13,14 Metabolic changes...

Outcome of Splenectomy in JAK2 Inhibitor Treated Patients With Myelofibrosis: A Mayo Clinic Experience in 34 Consecutive Cases
To the Editor, The management of symptomatic splenomegaly in myelofibrosis (MF) was revolutionized by the discovery of the JAK2V617F mutation and subsequent development of JAK2 inhibitors (JAKi) [1-4]. Currently, four JAKi are approved by the Food and Drug Administration (FDA) for treatment of MF: ruxolitinib, fedratinib, pacritinib, and momelotinib which alleviate symptomatic splenomegaly and constitutional symptoms. In the COMFORT I (ruxolitinib vs.
The Influence of Area Deprivation Index on Surgical Outcomes in Pancreas Cancer
Surgical resection for pancreas ductal adenocarcinoma (PDAC) remains the mainstay of treatment. Regardless of enhanced survival rates, disparities in patient outcomes and experiences driven by the social determinants of health (SDH) persist. The area deprivation index (ADI) comprises 17 indicators related to education, employment, housing-quality, and poverty. We sought to examine the impact of ADI on surgical outcomes of PDAC patients.

Liver surgery for metastatic colorectal cancer: the surgical oncologist perspective
Neoadjuvant/conversion chemotherapy has emerged as an indispensable tool to achieve resectability of initially unresectable metastatic colorectal cancer and improves oncological outcomes. In parallel, surgical strategy has adopted a more aggressive treatment approach to achieve complete tumor clearance. However, chemotherapy affects liver function and combined with extensive liver resection, morbidity has increased, thereby compromising oncological outcome.
