Patient harms during delayed access to clinical genetics services. A systematic literature review and synthesis without meta-analysis (SWiM)
Abstract Delays in accessing clinical genetics services are increasing globally. This systematic review and Synthesis Without Meta-analysis investigated the types and frequency of harms during delayed access to clinical genetics services (PROSPERO CRD42024601622). We searched peer-reviewed literature across multiple databases and included Australian policies, white papers and government documents reporting patient harms. Certainty of evidence was assessed using GRADE.
Quality in Australian Health Care Study 2: a national imperative for safer care
Background: the Quality in Australian Health Care Study and its legacy The original Quality in Australian Health Care Study (QAHCS), published in the Medical Journal of Australia in 1995, remains the most cited article in that journal's history.1 It found that 16.6% of hospital admissions were associated with patient harm (adverse events), with about half felt to be preventable, placing patient safety firmly on the national agenda.1 The Minister for Human Services and Health deemed the...
Perceptions and Experiences of Consumer Representatives on Patient Safety Investigation Teams: A Qualitative Analysis
1 Introduction Over the last 30 years, there has been a growing understanding that the health care system can, and does, harm patients (adverse events) [1]. Large scale studies, across many countries, have found that approximately 10% of hospital admissions are associated with an adverse event [2].
When patients are harmed in hospital, issues aren’t always fixed to avoid it happening again
Recently, the media has reported several cases of serious “adverse events”, where babies, children and an adult experienced harm and ultimately died while receiving care in separate Australian hospitals. When a serious adverse event occurs, hospitals investigate what happened and why, and propose recommendations to reduce the risk of similar harm occurring again. About 1600 patient safety investigations are undertaken each year. And the stakes are high.

How HSSIB contributes to patient safety: a view from Australia
The contributions of HSSIB Trusts undertaking effective investigations can improve local clinical processes and therefore reduce adverse events re-occurring.
Improving health system responses when patients are harmed: a protocol for a multistage mixed-methods study
Methods and analysis The study has three research questions: How should we decide what to investigate and to what level, to maximise learning to improve patient safety—making the best use of the limited resources available? How can we ensure investigations are of sufficient quality to identify contributing factors and identify effective system improvements?
Birang Daruganora: a protocol for a qualitative study to elicit Aboriginal and Torres Strait Islander community views about cultural needs and experiences for a new Australian health facility
Methods The Aboriginal Health Liaison officers of the WSLHD supported by Australian Institute of Health Innovation researchers will lead the study. Initially, the project team will meaningfully engage with stakeholders and communities to participate in the project. Existing networks will be approached through community Elders, and we will link in with service providers and media to engage with community to recruit participants for facilitator-coordinated workshops and follow-up interviews.

‘Matching Michigan’: a 2-year stepped interventional programme to minimise central venous catheter-blood stream infections in intensive care units in England | BMJ Quality & Safety
Matching Michigan followed, and took place during, heightened media interest and policy initiatives focused on healthcare-associated infections and BSIs ( table 1 ) including the introduction by the Department of Health (DoH) in 2007 of best practice guidance on CVC insertion and management 18 through its multicomponent ‘Saving Lives’ programme.
Harms from discharge to primary care: mixed methods analysis of incident reports
In all, 598 out of 644 reports were included in the analysis; 46 incidents were excluded due to reports not describing an actual patient safety incident (n = 18), insufficient narrative detail (n = 24), or no free-text description available (n = 4). Cohen’s ? statistics were calculated for the primary incident type (chronologically closest to the outcome experienced by the patient) at ? = 0.79 (95% confidence intervals = 0.78 to 0.81).
Patient Safety Incidents Involving Sick Children in Primary Care in England and Wales: A Mixed Methods Analysis
These are extracts from the free-text descriptions of incidents provided by the incident reporters. The extracts have been edited by the authors to correct typographical errors and remove indecipherable text. Example 1. Chloramphenicol eye drops 0.5% were prescribed but chloramphenicol ear drops 10% were dispensed from the fridge. This occurred because the medication was dispensed in a hurry and the pharmacist did not spot the error when the second check was made.
‘Matching Michigan’: a 2-year stepped interventional programme to minimise central venous catheter-blood stream infections in intensive care units in England -- Bion et al. 22 (2): 110 -- BMJ Quality and Safety
Results Of 223 ICUs in England, 215 (196 adult, 19 paediatric) submitted data on 2479 of 2787 possible months and 147 (66%) provided complete data. The exposure rate was 438?887 (404?252 adult and 34?635 paediatric) CVC-patient days. Over 20?months, 1092 CVC-BSIs were reported. Of these, 884 (81%) were ICU acquired.

Safety Incidents in the Primary Care Office Setting
BACKGROUND: In the United Kingdom, 26% of child deaths have identifiable failures in care. Although children account for 40% of family physicians’ workload, little is known about the safety of care in the community setting. Using data from a national patient safety incident reporting system, this study aimed to characterize the pediatric safety incidents occurring in family practice.
