Abstract Polycystic ovary syndrome (PCOS) is a common and heterogeneous endocrine disorder that affects 11%–13% of women worldwide, with profound implications for fertility and long-term metabolic health.

This is a cross-sectional study of transgender women followed up at the endocrine outpatient clinic of the Transdisciplinary Gender Identity Program of Hospital de Clínicas de Porto Alegre, Brazil. We included participants who had been prospectively recruited by our research group and had serum samples stored at −80°C in a biorepository from previous studies [2,22]. The aliquots were collected at a single time point for each participant between 2016 and 2023.
