CCFs are rare coronary anomalies characterized by abnormal connections between a coronary artery and one of the cardiac chambers. These and abnormal connections, though uncommon, can have significant clinical implications, including the development of heart failure and non-ischaemic ischemic cardiomyopathy, as evidenced by the patient in this report [1].

Case presentation A 42-year-old Gilaks female patient presented with mild lower limb weakness. She had hypokalemia in the initial lab tests, so the patient's weakness was attributed to hypokalemia in several visits and finally, she was diagnosed with renal tubular acidosis. After three months, the patient's weakness increased and the symptoms of urinary frequency and urgency along with dysesthesia and pain were added.
