High-dose budesonide for early COVID-19
We were encouraged by the results of the PRINCIPLE trial, which in vulnerable individuals showed inhaled budesonide to confer a non-significant –25% (95% CI –45 to 3) relative reduction in the composite coprimary endpoint of hospital admission or death, with the number needed to treat being 50. Notably, the study had 90% power to detect a 50% reduction in the composite endpoint.
Adrenal insufficiency in patients taking benralizumab as corticosteroid sparing therapy
In The Lancet Respiratory Medicine, Andrew Menzies-Gow and colleagues 1 Menzies-Gow A Gurnell M Heaney LG et al. Oral corticosteroid elimination via a personalised reduction algorithm in adults with severe, eosinophilic asthma treated with benralizumab (PONENTE): a multicentre, open-label, single-arm study.
Double pyramid technique of transoral laser partial laryngectomy for radiorecurrent laryngeal cancer
Abstract Management of recurrent head and neck cancer is challenging. Surgical treatments for residual or radiorecurrent laryngeal cancer include total laryngectomy, open partial laryngectomy and transoral laser microsurgery (TLM). TLM has been shown to achieve good oncological and functional outcomes in radiorecurrent laryngeal cancer.
Not quite Pott’s puffy tumour
Pott’s puffy tumour (PPT) is characterised by a subperiosteal abscess associated with osteomyelitis of the frontal bone—a rare complication of frontal sinusitis, more common in the paediatric population. We describe a case mimicking PPT, where abscess extension was facilitated by previous surgery. Usually patients with PPT would be systemically unwell, but our patient, a 63-year-old Caucasian man, was systemically well with a large swelling of his forehead.
