What PIT count says about county homelessness numbers: We are making progress amid uncertainty Original
Quick Take Robert Ratner, director of Housing for Health for Santa Cruz County’s human services department, and Larry Imwalle, director of Homelessness Response & Community Programs for the City of Santa Cruz, interpret the point-in-time count numbers, which were released last week. The count showed fewer unhoused residents countywide, but a 31% increase in Santa Cruz. The numbers, they say, tell only part of the story.
Illnesses associated with ketosis including diabetic ketoacidosis during very low carbohydrate and ketogenic diets
1 INTRODUCTION Very low carbohydrate ketogenic diets (VLCKDs) are rising in popularity among people with metabolic diseases due to growing awareness of their benefits for glycaemic control, inflammation and weight loss. VLCKDs are characterized by reducing dietary carbohydrate intake to 20–50 g/day (about 4 to 10% of a 2000-kcal/day diet). After several days of VLCKD, glycogen is depleted, and the body breaks down protein and fat for energy.
Metformin Should Be Used to Treat Prediabetes in Selected Individuals
Commentaries In this issue of Diabetes Care, Dr. Mayer Davidson proposes that prescription of metformin for patients with prediabetes is inappropriate (1). We respectfully disagree. Hyperglycemia is a continuous risk factor for adverse health outcomes.
The Future of Health Insurance for Patients With Diabetes
This transcript has been edited for clarity. It's hard to believe that we're now 10 years out from the institution of the Affordable Care Act. Since its inception, the Republican side has been interested in seeing its demise. The question in 2020 is: Where are we vis-à-vis the Affordable Care Act? Some Background One of the very first formal efforts to get rid of it was the Graham-Cassidy-Heller-Johnson bill that was presented in the Senate back in September 2017.

SGLT Inhibitors for Type 1 Diabetes: Proceed With Extreme Caution
Commentary Intensive insulin management is currently the only option for effective treatment of type 1 diabetes. Recent data from the T1D Exchange (T1DX) registry (1), which comprises leading U.S. diabetes treatment centers, show that despite the widespread availability of insulin analogs and increasing use of insulin pumps and continuous glucose monitoring systems, only about 20% of adult patients achieve the A1C target of <7% (53 mmol/mol) recommended by the American Diabetes Association (2).
Metformin and Type 2 Diabetes Prevention
Abstract IN BRIEF Metformin, an established therapy for the treatment of type 2 diabetes, has demonstrated safety and effectiveness in the prevention or delay of type 2 diabetes in people at high risk. The Diabetes Prevention Program randomized clinical trial demonstrated that intensive lifestyle intervention and metformin therapy reduced progression to diabetes by 58 and 31%, respectively, compared to placebo in people at risk of type 2 diabetes.
Should We Screen for Type 2 Diabetes?
We read with interest, concern, and ultimately outrage the argument made by Rind in Smetana and colleagues' grand rounds discussion ( 1 ) that not only is screening for diabetes a waste of time and resources but that “diabetes” (his use of quotation marks) is not itself a disease state. His contention that diabetes is not a disease worth screening for and treating is an astonishing position to take in the 21st century.
