Complementary surveillance strategies are needed to better characterise the epidemiology, care pathways and treatment outcomes of tuberculosis in children
In this TB-endemic setting, our study identified a very large burden of childhood TB managed at referral hospital level, with almost 400 children during a one-year period at a single hospital. Clinical surveillance identified 237 (60%) children in addition to the existing laboratory surveillance. Despite the majority of children being young (74.9% < 5 years of age), the diagnosis was bacteriologically (culture) confirmed in 40% of children.
Has universal screening with Xpert® MTB/RIF increased the proportion of multidrug-resistant tuberculosis cases diagnosed in a routine operational setting?
Primary health services in Cape Town, South Africa where the introduction of Xpert® MTB/RIF (Xpert) enabled simultaneous screening for tuberculosis (TB) and drug susceptibility in all presumptive cases. To compare the proportion of TB cases with drug susceptibility tests undertaken and multidrug-resistant tuberculosis (MDR-TB) diagnosed pre-treatment and during the course of 1st line treatment in the previous smear/culture and the newly introduced Xpert-based algorithms.
