Background Bleeding after cardiac surgery is common and continues to require 10–20% of the national blood supply. Transfusion of allogeneic blood is associated with increased morbidity and mortality. Excessive protamine in the absence of circulating heparin after weaning off CPB can cause anticoagulation and precipitate bleeding. Hence, adequate dose calculation of protamine is crucial yet under evaluated. Study Design Retrospective cohort study.

Acute normovolemic hemodilution (ANH) is a blood conservation strategy well-known to reduce allogeneic blood transfusion. More specifically, perioperative autologous whole blood collection and reinfusion during cardiac surgery has been shown to reduce allogeneic red blood cell (RBC) transfusions (4% vs. 44% in a control group).[1] In addition, the use of ANH has the potential to improve coagulation parameters, thereby reducing the administration of clotting factors and other blood products.
