1 Introduction C2 pedicle screw (C2PS) placement is a critical component of posterior fixation at the craniovertebral junction. In 2001, Harms and Melcher, building on the technique of Goel and Laheri, introduced an atlantoaxial posterior screw-rod fixation and fusion technique that established the C2 pedicle screw as the gold standard for C2 fixation [1, 2]. The C2PS traverses all three columns of C2, providing strong fixation strength and reduction capability.
