Hematopoietic cell transplantation (HCT) is a curative treatment for a number of benign and malignant hematologic diseases [1]. A key component of hematopoietic stem cell transplantation is prophylaxis against graft-versus-host disease. Since the end of the 1970 s, with the introduction of cyclosporine, calcineurin inhibitors (cyclosporine and tacrolimus) have become part of almost all prophylactic regimens, despite their poor toxicity profile and the need for monitoring.

1 Introduction Tacrolimus (TAC) is a calcineurin inhibitor (CNI) widely used for prophylaxis of graft-versus-host disease (GVHD) following allogeneic hematopoietic cell transplantation (HCT). It is commonly administered in combination with methotrexate (MTX), a regimen that has long been established as a standard approach for GVHD prevention [1, 2].
