Abstract Poor-quality healthcare contributes to preventable mortality, particularly in low- and middle-income countries. We conducted a parallel-group randomised controlled trial in Indonesia, Kenya, and the Netherlands to evaluate whether large language model (LLM) access improves physician clinical performance across economically diverse settings. Participants (N=249) were randomly assigned via simple randomisation to control or intervention (GPT-4o access).
