Dear Editor, Histoplasmosis, caused by Histoplasma capsulatum (dimorphic fungi), is a fungal infection that primarily involves the lungs and may disseminate to multiple organs in immunocompromised individuals [1]. The pathogenic form (yeast) of H. capsulatum can cause acute disease, or become latent. When H. capsulatum enters macrophages, it can avoid host effector responses and multiply and disseminate throughout the host organs.

The passing of Jishnu Sen today, and that of Samir Gangahar last year, have deleted a whole section in my life’s book, and I cannot write about one without including the other, because they came into my life together, back in 1989 in HTA Calcutta. They had just graduated, and within a couple of days after we met, we became colleagues, and very soon after, friends. They were by far the youngest in the office. I was a Calcutta tyro, going there with the trepidation of sailing in uncharted waters.

To the Editor: We read with interest the study by Perry et al1 on risk-stratified surveillance after piecemeal EMR of large (≥20 mm) nonpedunculated colorectal polyps (LNPCPs). In their 193-patient cohort, adenoma recurrence at first follow-up was significantly greater at 6-month versus 12-month surveillance (31.5% vs 14.3%), and the authors concluded that extending initial surveillance to 12 months in low-risk cases was safe and avoided unnecessary procedures.
