Graves’ disease: moving forwards
Antithyroid medication The ATD used to treat GD in children is the thionamide carbimazole (CBZ) or its active metabolite methimazole. CBZ does not need to be given more than once daily. Propylthiouracil (PTU) was previously used but should now be avoided in children because of the risk of liver failure.21 A framework for managing patients with ATD, including recommended starting doses,6 is presented in box 1.

Investigation and management of tall stature
At the population level, similar numbers of children have a height greater than 2 SD above the mean as those who have a height 2 SD below the mean. Despite this, a referral for the assessment of tall stature is much less common than a referral for short stature, highlighting societal perceptions of what is and what is not acceptable. Most tall children are healthy and simply have a familial tendency to tallness but there are a number of important pathological causes that need to be considered.
Investigation and management of short stature
Only a minority of short children, which can be defined as a height less than 2 standard deviation score (SDS) below the mean (less than the 2nd centile), will be found to have an underlying pathology. A diagnosis can frequently be reached without the need for further investigation, and this article will focus on some of the key features that indicate that a child is well and can be discharged.
Graves’ disease. Time to move on
Graves’ disease remains a challenge for a young person, their family and the healthcare team. There have been no new treatment options for the newly presenting patient for over 55 years1 and the reality is that most young people will ultimately become adults who take long-term thyroid hormone replacement. This is because most young people will relapse after a course of antithyroid drug (ATD)—irrespective of whether this is of 2, 4 or 6 years’ duration.
Who needs thyroid function testing at birth?
There have been notable advances in terms of what biochemistry laboratories can measure, how quickly they can measure and how much sample is required to do the measuring. This has improved many aspects of clinical care but ease of access and rapid feedback may increase the likelihood of babies undergoing blood tests unnecessarily. Checking thyroid-stimulating hormone (TSH) and thyroid hormone concentrations in neonates is a good case in point.
