To the Editor, We read with interest the Brief Report by Gray et al., which used STOP-FALLS data to describe medication substitutions after discontinuation of central nervous system (CNS)-active medications in older adults in primary care [1]. The authors address an underexamined question in deprescribing research: whether discontinuing a high-risk medication reduces overall medication-related risk or is followed by risk-preserving substitution.
