PCRTO protocol PCRTO had been adopted as a protocolized strategy in weaning patients from V-A ECMO support at our institution (Appendix 1). PCRTO was achieved by reducing the pump head revolutions in a controlled manner until a retrograde flow of 0.5–1.0 L/min (10 mL/kg/min) is attained.11 The distal reperfusion catheter was disconnected from the ECMO circuit and connected to a pressurized normal saline infusion to maintain its patency and allow continuous pressure monitoring (Figure 1).
