A 69-year-old male Chinese farmer presented with bilateral facial lesions that had gradually worsened during the previous 20 years. He had a history of smoking (>10 cigarettes every day) for at least 30 years, but was otherwise healthy and denied any exposure to radiation, toxins, or chemicals. On physical examination, multiple solitary open black comedones were located bilaterally on the malar cheeks and temples.
