Ambulatory Infusion Decision in Oncology: A Strategic Framework for Health System Leaders
The Market Is Moving Without You 1 Centers for Medicare & Medicaid Services (CMS) expanded its Home Infusion Therapy Services benefit in 2025, increasing reimbursement for professional services delivered outside the hospital.3 Private equity-backed platforms are accelerating consolidation: the recent merger of AleraCare and Pure Infusion Suites created a 75-site ambulatory infusion network across 14 states, positioned explicitly to absorb volume that payers redirect away from hospital...
Towards a decentralized future for open-science databases
|
Large-scale biological data repositories, particularly when deployed as single, non-mirrored instances or governed within narrow contexts, face structural vulnerabilities, from cyberattacks to funding disruptions. We propose a hybrid framework that integrates federated and decentralized models to ensure the resilience, sustainability and FAIR/CARE stewardship of scientific data as a global public good.

Extended aromatase inhibitor therapy for early breast cancer
A meta-analysis by the Early Breast Cancer Trialists’ Collaborative Group (EBCTCG) provides an important synthesis of the available randomised evidence on extended aromatase inhibitor therapy after at least 5 years of previous endocrine treatment, confirming a proportional reduction in recurrence.1 Although we commend the EBCTCG for their work, we highlight two considerations regarding the application of these findings in contemporary clinical practice.
Modern Diets, Metabolic Inequity, and Race-Ethnic Disparities: Unraveling the Associations With MASLD and Cancer Risk
1 Background The rising incidence of metabolic dysfunction–associated steatotic liver disease (MASLD) should be alarming [1], as it represents a significant public health emergency, driven in part by the massive rise in human consumption of processed and ultraprocessed foods (UPFs) [2]. Diets high in UPF have been linked to obesity, diabetes, and hyperlipidemia.
Reassessing Clinical Relevance of Cannabidiol-Associated Liver Enzyme Elevations
To the Editor We read with interest the recent study by Florian et al1 that investigated the effects of daily cannabidiol (CBD) use on liver enzymes and endocrine hormones. The study addresses a salient public health question; however, several issues merit further consideration to contextualize the findings.
Adverse Liver and Renal Outcomes After Initiating SGLT‐2i and GLP‐1RA Therapy Among Patients With Diabetes and MASLD
In this population-based cohort study, SGLT2 inhibitors (SGLT2i) were associated with significantly lower incidences of cirrhosis, hepatic decompensations (HR 0.84), and hepatocellular carcinoma (HCC) compared to DPP4i. Patients on SGLT2i demonstrated a notably reduced progression to chronic kidney disease (CKD) and severe CKD stages, as well as a lower need for hemodialysis compared to DPP4i users.
Uncovering the Structural and Functional Implications of Uncharacterized NSPs and Variations in the Molecular Toolkit Across Mammalian and Non-Mammalian Arteriviruses
Abstract Despite considerable scrutiny of mammalian arterivirus genomes, their genomic architecture remains incomplete, with several unannotated non-structural proteins (NSPs) and the puzzling absence of methyltransferase (MTase) domains. Additionally, the host range of arteriviruses has expanded to include seven newly sequenced genomes from non-mammalian hosts, which remain largely unannotated and await detailed comparisons alongside mammalian isolates.

Cardiovascular Outcomes and Mortality After Bariatric Surgery in Patients With Nonalcoholic Fatty Liver Disease and Obesity
Key Points Question Is bariatric surgery associated with the incidence of major adverse cardiovascular events or all-cause of mortality among patients with nonalcoholic fatty liver disease (NAFLD) and obesity?
Combined Endoscopic-Percutaneous Rendezvous for Biliary Continuity for Restoration of Completely Transected Common Bile Duct
Bile duct injuries (BDIs) remain a serious complication after open or laparoscopic cholecystectomy.A partial BDI can usually be managed by a combination of percutaneous and/or endoscopic techniques; however, managing a complete transection is particularly challenging.Stent placement is difficult in most cases because of the misalignment of the transected ends.
Use of a novel endoscopic tack and suture system for the management of pancreatocolonic fistula
Use of a novel endoscopic tack and suture system for the management of pancreatocolonic fistula 1 Fistulization in the GI tract in acute pancreatitis. Pancreatocolonic fistulae (PCF) are the rarest, with an incidence of 3%. Development of these fistulae is associated with significant morbidity, and when needed, their closure has conventionally been achieved by surgery. We present a patient with walled-off pancreatic necrosis (WOPN) complicated with PCF that was closed by a novel endoscopic method.
Validation of the accuracy of the FAST™ score for detecting patients with at-risk nonalcoholic steatohepatitis (NASH) in a North American cohort and comparison to other non-invasive algorithms

Kiwi genome provides insights into evolution of a nocturnal lifestyle
: Table S1). After read correction [ ] we assembled contigs and scaffolds using SOAPdenovo [ ] (Additional file : Note: Filtering and read correction; Genome assembly) to generate a draft assembly, which spanned 1.595 Gb (Additional file : Tables S2 and S3). The N50s of contigs and scaffolds were 16.48 kb and 3.95 Mb, respectively (Additional file : Table S3).
Kiwi genome provides insights into evolution of a nocturnal lifestyle
We prepared 11 libraries with several insert sizes from Apteryx mantelli genomic DNA and sequenced 83 billion base pairs (Gb) from small insert-size libraries and 120 Gb from large-insert mate-pair Illumina libraries (Additional file 1: Table S1). After read correction [5] we assembled contigs and scaffolds using SOAPdenovo [6] (Additional file 1: Note: Filtering and read correction; Genome assembly) to generate a draft assembly, which spanned 1.595 Gb (Additional file 1: Tables S2 and S3).
