Participants randomised to the control group will receive standard care based on local treatment guidelines but will not be permitted to receive cholecalciferol or ergocalciferol > 1000 IU per day for the duration of the study, unless vitamin D deficiency-related osteomalacia is diagnosed. Patients randomised to receive cholecalciferol will be prescribed cholecalciferol 60,000 IU fortnightly, in addition to standard care.
