1 Introduction Electroconvulsive therapy (ECT) involves administering electricity to the brain, under general anaesthesia, six to 12 times over several weeks, to produce tonic–clonic seizures. The clinical team usually involves mental health nurses, a psychiatrist, and an anaesthetist.

Most people are aware that ECT causes memory problems; since 2018 it has been literally written on the ECT machine itself following FDA concerns. Our own survey found that most ECT recipients and their relatives reported memory loss. But do recipients report any other adverse effects from ECT? Little was known, as previous studies were mostly miniature (e.g. 11 of the 16 studies to date involved 20 or fewer people).

Abstract Research into the safety of electroconvulsive therapy (ECT) usually focuses on memory loss. Studies asking patients directly reveal a broader range of adverse effects. This paper reports the responses of 747 ECT recipients and 201 relatives/friends, from 37 countries, to a question about 25 possible adverse effects, in an online survey. Seventeen of the 25 were reported by more than half of both the ECT recipients and the relatives/friends.
