Hepcidin controls iron concentration in blood plasma as well as organismal iron storage. The hormone acts by impeding iron delivery to plasma from iron-absorbing duodenal enterocytes, iron-storing hepatocytes, and macrophages, that recycle iron from aged erythrocytes. Decreasing hepcidin mobilizes iron for erythropoiesis or recovery from iron deficiency, while increased hepcidin causes hypoferremia, an effector of host defense.

When iron stores are low, gut metal ion transporters are upregulated to increase intestinal absorption of dietary iron. However, this mechanism can also increase absorption of cadmium in the diet.
