I have been in the United Kingdom for almost two years now, but during that time I have worked almost exclusively in community psychiatry.

Early in my career as a psychiatry medical officer in Malaysia, I assessed a young woman who had miscarried only hours earlier. She had arrived at the emergency department after an ultrasound confirmed what she already knew deep in her bones: The baby she carried was no more. Because the miscarriage was incomplete, she remained overnight while awaiting admission to the obstetrics and gynecology ward for surgery. The following morning, she awoke screaming and crying.
