Acquired aplastic anemia (aAA) an immune-mediated bone marrow failure, where the balance between curative hematopoietic stem cell transplantation (HSCT) and immunosuppressive therapy (IST) continues to evolve. In children lacking a matched related donor (MRD), horse antithymocyte globulin plus cyclosporine (CSA) remains the standard first-line therapy, achieving 70% hematologic responses and >80% overall survival (OS) at 5 years [1, 2].
