Cytogenetics-based clinical trajectories in patients with MRD negativity post autologous transplant for multiple myeloma
|
Abstract Measurable residual disease (MRD) assessment has become central to risk-adapted treatment strategies in multiple myeloma, yet the prognostic value of MRD negativity following autologous stem cell transplantation (ASCT) across individual high-risk cytogenetic abnormalities (HRCAs) remains unclear. We analyzed 351 newly diagnosed patients who achieved MRD negativity (sensitivity 2.4 × 10−6) between day 60-100 following ASCT and had baseline cytogenetic data available.

Combination of daratumumab, ixazomib, and lenalidomide with or without dexamethasone for initial therapy of newly diagnosed myeloma
|
Abstract Quadruplets incorporating anti-CD38 monoclonal antibody have led to improved outcomes, potentially allowing limited-duration therapy and reduction in steroid doses. We designed this trial to examine the efficacy of the quadruplet regimen containing daratumumab, ixazomib, lenalidomide, and dexamethasone (Dara-IRd) given for fixed duration and to evaluate early discontinuation of steroids. Patients with untreated MM were enrolled, irrespective of their transplant eligibility.

Characteristics and outcomes of secondary hematological malignancies following autologous stem cell transplantation for multiple myeloma
|
Abstract Autologous stem cell transplantation (auto-SCT) remains an important pillar in multiple myeloma (MM) therapy; however, there is an increased risk of developing second primary malignancy including second hematological malignancy (SHM). We performed a retrospective study of patients with MM who underwent an auto-SCT at our institution between 1990 and 2022 and subsequently developed SHM.

Gene expression-based dissemination score predicts early spread and poor outcomes in multiple myeloma
Abstract Multiple myeloma progresses from bone marrow–confined disease to systemic forms through processes linked to loss of adhesion and EMT-related programs. We analyzed baseline plasma cell RNA-seq, microarray, and single-cell datasets to derive and validate a dissemination score.
Elevated non-clonal bone marrow plasma cell fraction at diagnosis is associated with improved outcomes in multiple myeloma - Blood Cancer Journal
|
Abstract In newly diagnosed multiple myeloma (NDMM), current risk stratification relies on cytogenetics and disease burden, but emerging immune parameters may refine prognosis. While a higher non-clonal plasma cell fraction (NCPF) predicts favorable outcomes in smoldering myeloma, its relevance in NDMM remains unexplored.

Clinical course, risk factors and mitigating strategies for Immune effector cell-associated late onset neurotoxicities after ciltacabtagene autoleucel CAR-T in multiple myeloma | Blood Cancer Journal
Abstract We examined the clinical course and risk factors for late onset neurotoxicities, including nerve palsies (IEC-NP) and parkinsonism (IEC-PKS), in patients with relapsed/refractory multiple myeloma (RRMM) treated with ciltacabtagene autoleucel (cilta-cel) in standard-of-care practice (SOC). Among 235 RRMM patients who received cilta-cel, 15 (6.4%) developed IEC-NP and 9 (3.8%) developed IEC-PKS with one patient developing both.
Impact of t(11;14) primary cytogenetic abnormality and the cumulative effect of multiple high-risk cytogenetics at diagnosis on the outcomes of patients with primary plasma cell leukemia | Blood Cancer Journal
|
To the Editor: Primary plasma cell leukemia (pPCL) is a rare and aggressive variant of multiple myeloma (MM), marked by elevated clonal plasma cells (PCs) in the peripheral blood, accounting for ~1–2% of new MM cases [1]. Historically, it was defined as ≥20% circulating plasma cells (cPCs) and/or an absolute PC count >2 × 10⁹/L, but the International Myeloma Working Group (IMWG) revised this in 2013 to require only one of these criteria [2].

Refining PET-based response in extramedullary multiple myeloma using total lesion glycolysis - Leukemia
Abstract Positron Emission Tomography (PET)-based assessments are an integral part of response assessment in patients with multiple myeloma (MM) with extramedullary disease (EMD), yet their utility in EMD remains to be systematically studied. We retrospectively evaluated 95 patients with EMD undergoing FDG PET/CT imaging for metabolic response assessment using visual Deauville Scores (DS) and total lesion glycolysis (TLG).
Value of serum-free light chain measurements in response and progression assessment in multiple myeloma with monoclonal protein measurable by electrophoresis | Blood Cancer Journal
Abstract Uniform assessment of response to treatment is crucial to managing multiple myeloma (MM) and developing new therapies. Measurement of monoclonal protein forms the cornerstone of disease assessment in MM. According to International Myeloma Working Group (IMWG) guidelines, serum-free light chain (sFLC) is included in MM response assessment in patients with no measurable disease by electrophoresis and to define stringent complete response.
Lymphocyte Kinetics and Outcomes of Chimeric Antigen Receptor T Cell Therapy in Multiple Myeloma With Out of Specification Products
To the Editor, The treatment of multiple myeloma has changed dramatically over the last two decades, fueled by a better understanding of disease biology and the identification of novel targets that led to the development of highly active therapeutics including monoclonal antibodies, bispecific antibodies and chimeric antigen receptor T cells (CAR-T) [1-6]. Except for CAR-T cells, all other therapeutics are manufactured and ready for use when the patient needs them.
Interleukin-6 is a highly prognostic biomarker for POEMS syndrome - Leukemia
To the Editor: POEMS syndrome (Polyneuropathy, Organomegaly, Endocrinopathy, M-protein, and Skin changes) is a rare paraneoplastic clonal plasma cell disorder which remains a diagnostic challenge due to its diverse clinical manifestations [1]. POEMS can lead to interleukin-6 (IL-6) significant morbidity and multiorgan dysfunction [1].

The ratio of brain to liver glucose activity and disease activity in multiple myeloma - Blood Cancer Journal
Abstract Changes in metabolic activity in tumor cells is one of the hallmarks of cancer. Cancer cells exhibit the Warburg effect with high glucose consumption for their energy needs. This provides the basis for imaging using 18F-2-deoxy-D-glucose for positron emission tomography to assess tumor burden and response to therapy.
Venetoclax-based treatment combinations in relapsed/refractory multiple myeloma: practice patterns and impact of secondary cytogenetic abnormalities on outcomes | Blood Cancer Journal
|
Abstract Venetoclax (Ven), a BCL-2 inhibitor, has demonstrated efficacy in patients with relapsed/refractory multiple myeloma (RRMM) harboring a t(11;14) and/or elevated BCL-2 expression. However, data from clinical trial remain inconclusive. This retrospective study evaluated the efficacy and safety of Ven-based therapies in 232 MM patients without concurrent AL amyloidosis treated at Mayo Clinic sites between Jan 2015 and Dec 2023.

Implications of lymphocyte kinetics after chimeric antigen receptor T cell therapy for multiple myeloma - Leukemia
To the Editor Therapy for relapsed and refractory multiple myeloma (RRMM) has been revolutionized with the availability of chimeric antigen receptor T cell therapy (CAR-T) [1,2,3,4,5,6]. Currently idecabtagene vicleucel (Ide-Cel) [2, 3] and ciltacabtagene autoleucel (Cilta-cel) [4, 5], are approved by the Food and Drug Administration (FDA). Both products result in rapid and deep responses and improve overall survival in patients with RRMM.
Clonal plasma cell proportion in the synthetic phase identifies a unique high-risk cohort in multiple myeloma - Blood Cancer Journal
Abstract Risk stratification models based on cytogenetics and disease burden help identify high-risk patients with newly diagnosed multiple myeloma (NDMM). However, some high-risk patients remain undetected. This study evaluated the prognostic utility of the proportion of clonal plasma cells in the S-phase of the cell cycle for NDMM. Patients with active multiple myeloma diagnosed between 01/01/2013 and 01/31/2023 who underwent S-phase assessment were included.

September 5 Reviews
It’s invariably disappointing when a movie you’ve been looking forward to seeing doesn’t live up to expectations. But such is the case with the latest offering from writer-director Tim Fehlbaum. This re-creation of the day when Black September terrorists took members of the Israeli Olympic team hostage at the 1972 Munich Summer Games, sadly, misses the mark on many fronts.

Real-life sensitivity of flow cytometry minimal residual disease assessment for plasma cell neoplasms - Blood Cancer Journal Original
To the Editor: Multiple myeloma (MM), a malignancy of mature plasma cells, is considered the second most common hematological malignancy in the United States [1]. While recent advances in therapeutic options have significantly prolonged progression-free and overall survival, MM remains an incurable disease [2].
A Validated Composite Organ and Hematologic Response Model for Early Assessment of Treatment Outcomes in Light Chain Amyloidosis
Newly diagnosed AL amyloidosis patients were evaluated to develop a model for early assessment of treatment benefit at 6 months, integrating both hematologic (HR) and organ response (OR) assessment (testing cohort, Mayo: n = 473; validation cohort, Pavia: n = 575). Multiple OR were assessed as follows: All OR (AOR): response in all organs, mixed OR (MOR): response in some organs, no OR (NOR)].

Single cell multiomic analyses reveal divergent effects of DNMT3A and TET2 mutant clonal hematopoiesis in inflammatory response
Abstract DNMT3A and TET2 are epigenetic regulators commonly mutated in age related clonal hematopoiesis (CH). Despite having opposed epigenetic functions, these mutations are associated with increased all-cause mortality and a low risk for progression to hematological neoplasms. While individual impacts on the epigenome have been described using different model systems, the phenotypic complexity in humans remains to be elucidated.
Treatment patterns for AL amyloidosis after frontline daratumumab, bortezomib, cyclophosphamide, and dexamethasone treatment failures - Leukemia
The immediate goal for therapy in patients with systemic immunoglobulin light chain amyloidosis (AL) is to swiftly achieve at least a hematologic very good partial response (VGPR), given the consistently improved organ responses and survival with achievement of deep hematologic responses [1,2,3].
Outcomes of patients with multiple myeloma refractory to standard dose vs low dose lenalidomide - Blood Cancer Journal
Abstract Refractoriness to lenalidomide is an important factor determining the choice of therapy at first relapse in multiple myeloma (MM). It remains debatable if resistance to lenalidomide varies among MM refractory to standard doses vs low dose maintenance doses. In this study, we assessed the outcomes with subsequent therapies in patients with MM refractory to standard dose vs low dose lenalidomide.
Mode of progression in smoldering multiple myeloma: a study of 406 patients - Blood Cancer Journal
Abstract The approach to patients with high-risk smoldering multiple myeloma (SMM) varies among clinicians; while some advocate early intervention, others reserve treatment at progression to multiple myeloma (MM). We aimed to describe the myeloma-defining events (MDEs) and clinical presentations leading to MM diagnosis among SMM patients seen at our institution. We included 406 patients diagnosed with SMM between 2013–2022, seen at Mayo Clinic, Rochester, MN.
Muscle and fat composition in patients with newly diagnosed multiple myeloma - Blood Cancer Journal
Abstract Measures of muscle and adipose tissue mass have been associated with outcomes in several malignancies, but studies in multiple myeloma (MM) are inconsistent. The aim of this study was to evaluate the association between muscle and fat areas and radiodensity, and overall survival (OS) in patients with newly diagnosed MM. We included 341 patients diagnosed with MM from 2010–2019 who had an 18F-fluorodeoxyglucose positron emission tomography/computed tomography at diagnosis.
September 5 (2025) - Movie
September 5 Review Riveting ‘September 5’ Captures a Harrowing Moment in History. The utterly gripping docudrama ‘September 5’ recreates how ABC Sports covered a heinous, world-changing act of terrorism at the 1972 Summer Olympics. We take the idea of “breaking news” almost for granted now; the slogan is overused to the point that it’s almost laughable when a cable news network flashes “breaking news” on its chyron every time a politician drops a fork or ties their shoe.

Long-term outcomes of allogeneic stem cell transplant in multiple myeloma - Blood Cancer Journal
Abstract Allogeneic stem cell transplant (allo SCT) for multiple myeloma (MM) is potentially curative in some, while toxic in many others. We retrospectively analyzed 85 patients diagnosed with MM who underwent allo SCT as frontline or salvage therapy between 2000 and 2022 at Mayo Clinic Rochester and examined patient outcomes and prognostic markers.
Conditional survival in multiple myeloma and impact of prognostic factors over time - Blood Cancer Journal
Abstract Overall survival estimates from diagnosis are valuable for guiding treatment, but do not consider the years already survived. Conditional survival (CS) provides dynamic survival predictions over time. This study was conducted to estimate CS at 1–8 years from diagnosis and the impact of baseline prognostic factors on CS in multiple myeloma (MM) patients. This is a retrospective study including 2556 MM patients diagnosed between 2004 and 2019.
Utility of flow cytometry screening before MRD testing in multiple myeloma - Blood Cancer Journal
TO THE EDITOR: Multiple myeloma (MM) is the second most common hematological malignancy in the United States predicted to cause 34,470 new cases and 12,640 deaths in 2022 [1]. Outcomes of patients with MM continue to improve with the advent of highly effective multidrug therapy regimens and high-dose chemotherapy with autologous stem cell transplant [2].

Defining drug/drug class refractoriness vs lines of therapy in relapsed/refractory multiple myeloma - Blood Cancer Journal
Dear Editor, Prior treatments in multiple myeloma (MM) have traditionally been described using lines of therapy (LOT) [1]. A LOT is defined as one or more complete cycles of a single/combination of agents, or a planned sequential therapy consisting of several regimens [1].
Thrombosis in multiple myeloma: Risk estimation by induction regimen and association with overall survival
Abstract Lenalidomide-containing (R) triplet and quadruplet regimens are the standard of care for multiple myeloma (MM) and have been shown to increase the risk of thrombosis. The association between thromboembolism (TE) and survival in the novel multidrug era is not yet delineated. In this study, we evaluated the incidence of TE during the first year of MM diagnosis, its association with the type of induction regimen, and its impact on overall survival.
Value of bone marrow examination in determining response to therapy in patients with multiple myeloma in the context of mass spectrometry-based m protein assessment | Leukemia
To the editor, Multiple Myeloma (MM) is a malignant plasma cell disorder secreting a monoclonal immunoglobulin [1]. The detection of this M-protein is central to the diagnosis and monitoring of multiple myeloma [2]. In 2006, the International Myeloma Working Group (IMWG) defined complete response (CR) as a negative immunofixation (IFE) on the serum and urine, and less than 5% plasma cells (PCs) on bone marrow examination (BME) [3].
