Abstract Continuous positive airway pressure (CPAP) remains the cornerstone of therapy for obstructive sleep apnea, yet its impact on preventing cardiovascular disease remains uncertain. Despite widespread clinical use, randomized controlled trials have not shown cardiovascular benefits with CPAP. Emerging evidence suggests that obstructive sleep apnea is a heterogeneous disease, and a uniform approach to treatment may obscure potential benefits or harm for individuals.

In the past five years, progress in obesity pharmacology has led to speculation that future obstructive sleep apnoea (OSA) treatment might not require the use of continuous positive airway pressure (CPAP). Treating obesity in patients with OSA is desirable, since addressing the underlying cause of the disease is generally recommended. Although weight loss can be efficacious for OSA,1 we believe that CPAP will be used for OSA treatment for the foreseeable future.
