Physicians and artificial intelligence diverge in evaluating large language models on real clinical cases
|
Abstract While multimodal large language models (LLMs) demonstrate significant potential in healthcare applications, their clinical utility is difficult to appraise. Current evaluations of medical-assisting LLMs are often limited by sparse human expertise, narrow specialty scope, and reliance on multiple-choice benchmarks or synthetic vignettes, which can inflate performance and obscure clinical utility.

Agentic AI in global health equity for high altitude populations
Abstract Successfully adapting to life in the highest altitudes (“Roof of the world”) is a heritage of evolutionary adaptation for humans. With rising interest in adventure travel and expanding transport networks that facilitate mobility from low to high altitudes, provision of healthcare for populations living in high-altitude regions has re-emerged as an area of interest and research. These populations have several unique characteristics that limit the simple generalization of medical knowledge.
Optic nerve cerebrospinal fluid drainage via lymphatic vessels in its sheath
Abstract Cerebrospinal fluid (CSF) surrounding the optic nerve is essential for nutrient transport, waste removal, and optic nerve function. Optic disc edema in astronauts during long-duration spaceflight—Spaceflight Associated Neuro-Ocular Syndrome (SANS)—may result from impaired optic nerve CSF drainage, but the drainage pathway remains unclear.
Robust and interpretable unit level causal inference in neural networks for pediatric myopia - npj Digital Medicine
Abstract Understanding causal mechanisms in deep learning is essential for clinical adoption, where interpretability and reliability are critical. Most existing AI systems act as black boxes, limiting transparency in medicine. We propose a causal inference framework integrated into neural networks to assess the influence of individual features on predictions.

Differences in Screening Indicator Performance for Primary Angle Closure and Primary Angle Closure Diseases: The Handan Eye Study
Keywords primary angle closure disease AS-OCT screening indicator Introduction Primary angle closure glaucoma (PACG) is estimated to affect approximately 32 million people globally by 2040, with three-quarters of cases occurring in Asia.1 Additionally, PACG is often associated with a high incidence of unilateral blindness upon initial diagnosis and carries a threefold greater risk of severe bilateral visual impairment than primary open-angle glaucoma does.2 Since the disease is preventable...
GWAS for primary angle-closure glaucoma identifies loci related to ocular biometry and morphology - Nature Communications
Abstract GWAS of primary angle-closure glaucoma have identified eight loci conferring risk in Asian populations. However, it remains unclear whether the genetic risk factors for the disease are consistent across different populations. Here, we present a discovery GWAS for primary angle-closure glaucoma in Europeans using the UK Biobank. We replicate our findings in six independent European populations and compare these results with results from 14 Asian cohorts.

Macular thickness and its associated factors in a healthy cynomolgus colony: The Non-human primates Eye study
Keywords Non-human primates Macular thickness associated factors Introduction As of 2010, the number of individuals with age-related macular degeneration (AMD) had reached twenty-three million(1). Meanwhile, AMD is the leading cause of blindness among people over 50 years of age in the United States(2). It is therefore important to focus on the prevention and treatment of age-related macular diseases in an aging society.

Non-human primates as preclinical models for optic nerve research: advancing insights into their application and potential - Eye
Abstract Optic neuropathies are a group of disorders characterized by damage or dysfunction of the optic nerve, which transmits visual information from the retina to the brain. Common causes include glaucoma, ischemic optic neuropathy, optic neuritis, hereditary optic neuropathies and traumatic or compressive optic neuropathies. These conditions can result in vision loss, decreased visual acuity, color vision defects, and visual field abnormalities.

Quantitative ultrasound image assessment of the optic nerve subarachnoid space during 90-day head-down tilt bed rest - npj Microgravity
Abstract The elevation in the optic nerve sheath (ONS) pressure (ONSP) due to microgravity-induced headward fluid shift is the primary hypothesized contributor to SANS. This longitudinal study aims to quantify the axial plane of the optic nerve subarachnoid space area (ONSSA), which is filled with cerebrospinal fluid (CSF) and expands with elevated ONSP during and after head-down tilt (HDT) bed rest (BR). 36 healthy male volunteers (72 eyes) underwent a 90-day strict 6° HDT BR.
Effects of Schlemm’s Canal Suture Implantation Surgery and Pilocarpine Eye Drops on Trabecular Meshwork Pulsatile Motion
1. Introduction Lowering the IOP remains the only effective treatment method for primary open-angle glaucoma (POAG), which is caused by elevated intraocular pressure (IOP) [1]. The main cause of elevated IOP is an increase in resistance to the outflow of aqueous humor. The main mechanism of the increase in aqueous humor outflow resistance in POAG is the increase in trabecular meshwork (TM) resistance and the collapse of Schlemm’s canal (SC) [2].

Intraocular Pressure Fluctuation in Primary Open-Angle Glaucoma with Canaloplasty and Microcatheter Assisted Trabeculotomy
Abstract : Background: Schlemm’s canal (SC) targeted procedures constitute a promising therapy for open angle glaucoma (POAG), safer and less invasive. However, little attention was paid to the intraocular pressure (IOP) variation in patients receiving these procedures, which is the risk factor for POAG progression. This study is to evaluate the IOP variation in eyes with POAG after modified canaloplasty (MC) and microcatheter assisted trabeculotomy (MAT).
