1 Introduction Monoclonal gammopathy of undetermined significance (MGUS) represents the near-universal precursor to multiple myeloma and reflects clonal plasma cell expansion under selective pressures that remain incompletely defined [1, 2]. Molecular analyses of immunoglobulin genes in plasma cell dyscrasias demonstrate somatic hypermutation patterns consistent with antigen-driven selection. These observations support a role for chronic antigenic stimulation in clonal evolution [3, 4].
