Prediction of Adjuvant Gemcitabine Sensitivity in Resectable Pancreatic Adenocarcinoma Using the GemPred RNA Signature: An Ancillary Study of the PRODIGE-24/CCTG PA6 Clinical Trial | Journal of Clinical Oncology
This ancillary study of a phase III randomized trial demonstrates that among the quarter of patients with a GemPred-positive transcriptomic signature, survival was comparable with that of mFOLFIRINOX, whereas those receiving adjuvant gemcitabine had fewer adverse events. Eighty-nine patients' tumors (25.5%) were GemPred+ and were thus predicted to be gemcitabine-sensitive.
Pancreatic ductal adenocarcinoma (PDAC) is an aggressive cancer, with only 11% of patients alive 5 years after their diagnosis, all stages considered.1 After surgery, adjuvant gemcitabine- or fluoropyrimidine-based monotherapies have been the standard since two decades.2-4 A modified folinic acid, fluorouracil, irinotecan, and oxaliplatin (mFOLFIRINOX) adjuvant regimen combining fluorouracil, leucovorin, irinotecan, and oxaliplatin has shown in the PRODIGE-24/CCTG PA6 trial to significantly...
Germline BRCA testing in pancreatic cancer: improving awareness, timing, turnaround, and uptake
Introduction Patients with pancreatic ductal adenocarcinoma (PDAC) face a particularly poor prognosis, with 5-year relative survival of 12.5%1 and 5-year actual survival of only 4.2%.2 However, for those with metastatic disease and a germline (i.e. heritable) BRCA1 or BRCA2 mutation (gBRCAm), first-line platinum-based chemotherapy followed by maintenance therapy with olaparib – a poly(adenosine diphosphate-ribose) polymerase (PARP) inhibitor – can significantly prolong progression-free...

Highlights • The role of gemcitabine-based adjuvant treatment in biliary tract cancer is uncertain. • The role of gemcitabine-based adjuvant treatment in biliary tract cancer is unproven. • We performed a combined analysis (n = 419) of the two recent trials BCAT and PRODIGE 12. • The analysis does not show benefit in recurrence-free survival or overall survival. • Gemcitabine-based adjuvant chemotherapy should not be used in biliary tract cancers.

A novel event-free survival endpoint in locally advanced pancreatic cancer
Introduction About 30–40% of patients with pancreatic ductal adenocarcinoma (PDAC) have a locally advanced pancreatic cancer (LAPC) which is non-metastatic and limited to the pancreatic region, but unresectable due to involvement of major blood vessels.1 The recommended management in eligible patients consists of chemotherapy with 5-fluorouracil, irinotecan and oxaliplatin (FOLFIRINOX) or gemcitabine/nab-paclitaxel (G/NP) combinations potentially followed by chemoradiation depending on...

FOLFIRINOX or Gemcitabine as Adjuvant Therapy for Pancreatic Cancer
23 References 419 Citing Articles Letters 1 Comment Related Articles Abstract Background Among patients with metastatic pancreatic cancer, combination chemotherapy with fluorouracil, leucovorin, irinotecan, and oxaliplatin (FOLFIRINOX) leads to longer overall survival than gemcitabine therapy. We compared the efficacy and safety of a modified FOLFIRINOX regimen with gemcitabine as adjuvant therapy in patients with resected pancreatic cancer.
Effect of Chemoradiotherapy vs Chemotherapy on Survival in Patients With Locally Advanced Pancreatic Cancer Controlled After 4 Months of Gemcitabine With or Without Erlotinib: The LAP07 Randomized Clinical Trial | Clinical Pharmacy and Pharmacology | JAMA
Importance In locally advanced pancreatic cancer, the role of chemoradiotherapy is controversial and the efficacy of erlotinib is unknown. Objectives To assess whether chemoradiotherapy improves overall survival of patients with locally advanced pancreatic cancer controlled after 4 months of gemcitabine-based induction chemotherapy and to assess the effect of erlotinib on survival.
