In newborns with congenital heart disease, approximately twenty percent have anomalies affecting the right ventricular outflow tract, such as tetralogy of Fallot with or without pulmonary atresia, requiring surgical repair with bioprosthetic valves, valved conduits or transannular patch. Reference Verheugt, Uiterwaal and van der Velde1 Severe pulmonary valve regurgitation is observed in almost half of this population immediately after the initial operation and in 85% after 2 years.

Direct Percutaneous Transthoracic Cardiac Access for Recanalization of Longstanding Branch Pulmonary Artery Atresia:

Abstract We describe the use of direct percutaneous cardiac access to recanalize an atretic right pulmonary artery in an adolescent with complex congenital heart disease and right heart failure. This case highlights the problems associated with loss of central venous access and potential advantages of a direct cardiac approach to catheterization.
