Abstract Behçet syndrome is a complex systemic immune-mediated form of vasculitis characterized by diverse clinical manifestations and a variable disease course. The treat-to-target (T2T) concept has emerged as a pivotal approach in managing various systemic autoimmune rheumatic diseases; however, its application in Behçet syndrome requires further work.

Methods This is a phase 3, multicentre, prospective, randomised, evaluator-blind, active-controlled, parallel-group study to evaluate the efficacy and safety of either IFX or ADA in patients with BS after induction therapy with systemic GC. Patients were recruited from four tertiary referral centres for BS in Italy (Pisa, Florence, Siena and Cagliari).
