For much of my career, I understood my responsibility in familiar terms: care for the patient in front of me, advocate for better health systems, and teach the next generation of trainees. I still believe that. But I now believe something else just as strongly: If we are serious about health, we cannot stop at the clinic door. We also have to show up in civic life and in the policy spaces that shape whether our patients can live healthy lives.

Why advocacy matters now more than ever

I had a patient once who told me that he never discloses his HIV status in the emergency department until after he’s established rapport with the physician. When I asked why, he explained that physicians too often treat patients living with HIV as their diagnosis and nothing more. This was his small but meaningful act of self-advocacy in a medical system that frequently places stigma and discrimination on marginalized patients.

He remains because losing SNAP benefits means returning to an empty apartment with no food. This isn't health care; it's a symptom of a system that forces hospitals to become shelters and physicians to witness preventable suffering.
