An otherwise healthy 59-year-old female presents with fecal urgency and bright red blood per rectum. On examination, she is found to have a palpable mass in the anal canal. Biopsies confirm an invasive moderately- to poorly-differentiated squamous cell carcinoma with diffuse p16 positivity on immunohistochemistry. Pelvic magnetic resonance imaging (MRI) demonstrates a 7.9 cm mass located 2.4 cm from the anal verge with invasion of the pelvic floor, internal anal sphincter, and cervix ( Fig. 1 ).
