To the Editor -In January 2023, the Science and Security Board of the Bulletin of the Atomic Scientists moved the hands of the Doomsday Clock forward to 90 seconds before midnight, reflecting the growing risk of nuclear war. In August 2022, the UN secretary general, António Guterres, warned that the world is now in "a time of nuclear danger not seen since the height of the Cold War". The danger has been underlined by growing tensions between many nuclear-armed states.

2026 marks an inflection point for the MJA in a number of ways. During 2025 we spent time considering how best to position the journal for the future. Some of this work relates to how the journal will look, and some of this relates to how we will manage our processes going forward. In doing this work, we had to balance several priorities, keeping in mind our primary purpose to publish high quality research and commentary that will inform health policy and influence medical practice in Australia.
