Hernández-Verdin et al. show that tumor-derived gamma-aminobutyric acid (GABA) contributes to immunotherapy resistance in TLS-positive tumors. Integrated spatial and functional analyses reveal that GABA induces TLS dysfunction and inhibits B cell activity via GABAB receptor interaction and metabolic reprogramming, while the inhibition of GABA synthesis enhances immune infiltration and therapeutic efficacy.

Abstract Overexpression of complement genes in the tumor microenvironment, including Factor H (FH), is a strong predictor of poor prognosis in multiple cancers. Its canonical functions in the bloodborne complement cascade, though, cannot explain this prognostic impact. Here, we demonstrate that FH operates within the intracellular space in fibroblasts and tumor cells.

Abstract Based on the demonstrated clinical activity of immune-checkpoint blockade (ICB) in advanced dedifferentiated liposarcoma (DDLPS) and undifferentiated pleomorphic sarcoma (UPS), we conducted a randomized, non-comparative phase 2 trial (NCT03307616) of neoadjuvant nivolumab or nivolumab/ipilimumab in patients with resectable retroperitoneal DDLPS (n = 17) and extremity/truncal UPS (+ concurrent nivolumab/radiation therapy; n = 10).
