Imagine a jumbo jet crashing every day, throughout the year, in a single country. The public would certainly be in an uproar, airlines would be grounded, and aviation authorities would intervene immediately. But, imagine another version of this scenario. Instead of planes crashing, it is patients dying due to medical errors that could have been prevented—resulting in a death toll of equivalent magnitude.
