Abstract Very high-risk neuroblastoma is induction-refractory and often harbors mutations in RAS and/or p53 signaling combined with telomere maintenance mechanisms. Event-free survival is <20% in these children. Patients unable to mobilize sufficient hematopoietic stem cells to harvest for busulfan/melphalan-based high-dose chemotherapy before autologous transplantation are also at high risk for relapse.

Nevertheless, the scientific evidence for the benefit of exercise therapy in pediatric oncology has led to the implementation of exercise therapy programs at most of the German hospitals.

1 Introduction Adaptive clinical trial designs for a single primary time-to-event endpoint are well established (see, e.g., Schäfer and Müller 2001; Wassmer 2006). These are based on the log-rank test by exploiting its independent increments structure as exhibited in Tsiatis (1981) and Sellke and Siegmund (1983) or in even broader generality by Scharfstein, Tsiatis, and Robins (1997).
