Key Points Question How have the burden of ischemic heart disease (IHD) mortality in the US and the influence of its risk factors changed between 1990 and 2023? Findings Per the results of this systematic analysis, there were an estimated 473 000 IHD deaths in the US in 2023, indicating the age-standardized mortality rate decreased by 58.7% since 1990. IHD deaths attributable to high body mass index (BMI) and high fasting plasma glucose (FPG) increased by 12.5% and 10.5% since 1990, respectively.

In Reply We thank Drs Rader and Brownstein for their thoughtful letter about our recent publication reporting estimates of US state-level adult obesity prevalence by race and ethnicity group through 2035. They correctly point out that our forecasts are based on past trends and raise the concern that our results do not account for the potential effect of recent changes in prescribing patterns of weight-management medications such as GLP-1 RAs on the prevalence of obesity.
