Teclistamab versus lenalidomide-dexamethasone in high-risk smoldering multiple myeloma: a randomized phase 2 trial
Abstract Teclistamab, a B cell maturation antigen-targeting bispecific antibody, has demonstrated substantial activity in relapsed multiple myeloma (MM), particularly in earlier lines of therapy, and may have higher efficacy in high-risk smoldering MM (HR-SMM) with a more functional immune system. In the randomized phase 2 ImmunoPRISM trial, we compared fixed-duration teclistamab with lenalidomide–dexamethasone (Rd) in HR-SMM.
Macrophage reprogramming, not CD8 T cell dynamics, characterizes durable disease control following PD-1 blockade in smoldering myeloma
|
Although PD-1 blockade has produced durable responses in a subset of patients with multiple myeloma (MM), no positive immune correlate has been established, and whether the active compartment is the CD8⁺ T cell axis canonical to solid tumors remains an open question [1,2,3]. Bone-marrow (BM) macrophages shape plasma-cell responses to immunomodulatory and checkpoint-based therapies, and recent profiling has shown that MM BM CD8⁺ T cells lack hallmarks of canonical exhaustion [4, 5].

Ciltacabtagene autoleucel in high-risk smoldering multiple myeloma: the CAR-PRISM phase 2 trial
|
Abstract High-risk smoldering multiple myeloma (HR-SMM) carries an increased risk of progression to multiple myeloma, making it an ideal setting to test whether chimeric antigen receptor (CAR) T cell therapy can achieve curative outcomes. Here in this phase 2 study, patients with HR-SMM received ciltacabtagene autoleucel (cilta-cel) at 0.3–0.5 × 106 or >0.5 × 106 viable CAR+ T cells per kilogram without induction or bridging therapy. Patients with >40% marrow involvement were excluded.

Daratumumab in high-risk MGUS and low-risk smoldering myeloma: results of the Phase II D-PRISM study
|
Abstract Daratumumab is approved for patients with multiple myeloma (MM) and high-risk smoldering MM (HR-SMM). However, HR-SMM is often as genomically complex as MM, suggesting it may be too advanced for single-agent intervention.

Integrating single-cell biophysical and transcriptomic features to resolve functional heterogeneity in mantle cell lymphoma
Abstract Intratumor heterogeneity impacts disease progression and therapeutic resistance but remains poorly characterized by conventional histologic, immunophenotypic, and molecular approaches. Single-cell biophysical properties distinguish functional phenotypes complementary to these approaches, providing additional insight into cellular diversity.

Evolutionary history of transformation from chronic lymphocytic leukemia to Richter syndrome - Nature Medicine
|
Abstract Richter syndrome (RS) arising from chronic lymphocytic leukemia (CLL) exemplifies an aggressive malignancy that develops from an indolent neoplasm. To decipher the genetics underlying this transformation, we computationally deconvoluted admixtures of CLL and RS cells from 52 patients with RS, evaluating paired CLL–RS whole-exome sequencing data.

Immune biomarkers of response to immunotherapy in patients with high-risk smoldering myeloma
Patients with smoldering multiple myeloma (SMM) are observed until progression, but early treatment may improve outcomes. We conducted a phase II trial of elotuzumab, lenalidomide, and dexamethasone (EloLenDex) in patients with high-risk SMM and performed single-cell RNA and T cell receptor (TCR) sequencing on 149 bone marrow (BM) and peripheral blood (PB) samples from patients and healthy donors (HDs).
Genetic subtypes of smoldering multiple myeloma are associated with distinct pathogenic phenotypes and clinical outcomes - Nature Communications
Abstract Smoldering multiple myeloma (SMM) is a precursor condition of multiple myeloma (MM) with significant heterogeneity in disease progression. Existing clinical models of progression risk do not fully capture this heterogeneity. Here we integrate 42 genetic alterations from 214 SMM patients using unsupervised binary matrix factorization (BMF) clustering and identify six distinct genetic subtypes.
Survival outcomes with warfarin compared with direct oral anticoagulants in cancer-associated venous thromboembolism in the United States: A population-based cohort study
Loading metrics Open Access Peer-reviewed Research Article ? This is an uncorrected proof. Abstract Background Direct oral anticoagulants (DOACs) have comparable efficacy with low-molecular-weight heparin (LMWH) for the treatment of cancer-associated venous thromboembolism (VTE). Whether there is a mortality benefit of DOACs compared with warfarin in the management of VTE in cancer is not established.
Prevalence of monoclonal gammopathies and clinical outcomes in a high-risk US population screened by mass spectrometry: a multicentre cohort study
Summary Background Prevalence estimates for monoclonal gammopathy of undetermined significance (MGUS) are based on predominantly White study populations screened by serum protein electrophoresis supplemented with immunofixation electrophoresis. A prevalence of 3% is reported for MGUS in the general population of European ancestry aged 50 years or older. MGUS prevalence is two times higher in individuals of African descent or with a family history of conditions related to multiple myeloma.
Everyone's responsibility: Students succeed when leaders talk technology
Education and technology have always gone hand in hand. However, when the COVID-19 pandemic hit and schools quickly had to make the switch to remote learning, the importance of technology within the education sector, including K-12 schools, was made even more evident. Technology adoption is changing access for students in today's hybrid learning environment.
Personal neoantigen vaccines induce persistent memory T cell responses and epitope spreading in patients with melanoma
Abstract Personal neoantigen vaccines have been envisioned as an effective approach to induce, amplify and diversify antitumor T cell responses. To define the long-term effects of such a vaccine, we evaluated the clinical outcome and circulating immune responses of eight patients with surgically resected stage IIIB/C or IVM1a/b melanoma, at a median of almost 4 years after treatment with NeoVax, a long-peptide vaccine targeting up to 20 personal neoantigens per patient (NCT01970358).
Chimeric Antigen Receptor T-Cell Therapy-Associated Cardiomyopathy in Patients With Refractory or Relapsed Non-Hodgkin Lymphoma
Adoptive cell therapy using chimeric antigen receptor (CAR) T cells augments the ability of the patient’s immune system to recognize and destroy cancer cells. Genetically modified autologous T-cell agents that target CD-19, tisagenlecleucel and axicabtagene ciloleucel, are currently approved for the treatment of children with B-cell acute lymphocytic leukemia and adults with aggressive B-cell lymphoma.
A peripheral immune signature of responsiveness to PD-1 blockade in patients with classical Hodgkin lymphoma
Abstract PD-1 blockade is highly effective in classical Hodgkin lymphomas (cHLs), which exhibit frequent copy-number gains of CD274 (PD-L1) and PDC1LG2 (PD-L2) on chromosome 9p24.1. However, in this largely MHC-class-I-negative tumor, the mechanism of action of anti-PD-1 therapy remains undefined.

Clonal hematopoiesis is associated with adverse outcomes in multiple myeloma patients undergoing transplant
Abstract Multiple myeloma (MM) is a plasma-cell neoplasm that is treated with high-dose chemotherapy, autologous stem cell transplant (ASCT) and long-term immunomodulatory drug (IMiD) maintenance. The presence of somatic mutations in the peripheral blood is termed clonal hematopoiesis of indeterminate potential (CHIP) and is associated with adverse outcomes.
