COMBI-I: Long-Term Overall Survival With Spartalizumab Plus Dabrafenib and Trametinib in BRAF V600–Mutant Advanced Melanoma
BRAF V600–mutant advanced melanoma.1 Patients with advanced melanoma, irrespective of driver aberrations in oncogenes, can be treated with immune checkpoint inhibitors.2 Previous studies have assessed the possibilities and outcomes of combining BRAF and MEK-targeted therapies with anti–PD-1/PD-L1 antibodies.3-13 The combination of atezolizumab with cobimetinib and vemurafenib is now approved by the US Food and Drug Administration as first-line treatment for unresectable advanced BRAF...
The concept of functional cure in advanced/metastatic melanoma treated with combined nivolumab and ipilimumab or nivolumab alone
Abstract Immune-Checkpoint-Inhibitors (ICIs) have significantly improved the course of advanced/metastatic melanoma and the concept of ICIs curative potential began to appear. To use statistical approaches to determine whether advanced/metastatic melanoma patients treated with nivolumab alone or in combination with ipilimumab may be considered as functionally cured. Individual patient data were reconstructed from published Kaplan-Meier curves of the anti-PD1 arms of the Checkmate-067.
Years of life lost in patients with a false-negative diagnosis of primary melanoma. A prospective study of the German Central Malignant Melanoma Registry involving 9063 patients over 28 years
|
Abstract A false-negative diagnosis of cancer can lead to a delay in effective treatment and a poorer prognosis. Here, we use the example of cutaneous melanoma to examine how many years of life are lost after a false-negative diagnosis compared to a primarily correct diagnosis. From 1996 to 2015, 9,063 patients are prospectively registered in the German Central Malignant Melanoma Registry and followed up until December 2023. A false-negative diagnosis is found in 206 (2.3%) patients.

Safety and efficacy of intratumoural anti-CTLA4 with intravenous anti-PD1
|
Abstract Intravenous administration of anti-CTLA4 with anti-PD1 provides durable tumour responses but causes severe treatment-related adverse events in patients with cancer1. Intratumoural administration at lower doses but high local concentrations could enhance antitumour efficacy while minimizing systemic exposure and toxicity.

Numerous retrospective studies and reviews have reported a positive association between immune-related adverse events (irAEs) and survival in non-small cell lung cancer (NSCLC) and melanoma patients treated with immune checkpoint blockers (ICBs). However, some results are controversial and the studies, whose results converge, should be interpreted cautiously because most of them do not deal appropriately with the immortal-time bias.

Randomized Phase III Trial Evaluating Spartalizumab Plus Dabrafenib and Trametinib for BRAF V600-Mutant Unresectable or Metastatic Melanoma
Systemic therapies have revolutionized the management of advanced melanoma, with significant improvements in overall survival (OS) observed in patients treated with both BRAF- and MEK-targeted therapies as well as immune checkpoint inhibitors.1-5 Despite these advancements, nearly half of the patients with unresectable or metastatic melanoma die within 5 years of starting treatment.
Multi-omics prediction in melanoma immunotherapy: A new brick in the wall
In this issue of Cancer Cell, Newell et al. introduce whole-genome and methylome data to melanoma immunotherapy response analysis. Genome breaks are more frequent in resistant tumors, but the best response classifiers remain mutation burden and interferon-ɣ signature. Clinical translation will need aggregation of many such datasets.
Positive Association Between Location of Melanoma, Ultraviolet Signature, Tumor Mutational Burden, and Response to Anti-PD-1 Therapy
Immune checkpoint inhibitors (ICIs), including programmed cell death-1 protein (PD-1)/programmed cell death-ligand-1 and cytotoxic T-lymphocyte–associated protein 4 (CTLA-4) inhibitors, have profoundly changed the prognosis of patients in multiple cancer types, including advanced melanoma.1,2 PD-1 inhibitors (pembrolizumab and nivolumab) have been approved by the US Food and Drug Administration for metastatic melanoma, and recently in the adjuvant setting in stage III melanoma.3,4 Although...

Long-term outcomes in patients with advanced melanoma who had initial stable disease with pembrolizumab in KEYNOTE-001 and KEYNOTE-006
Highlights • Prognosis of patients with early stable disease (SD) with pembrolizumab is unclear. • Results showed that response or SD at weeks 12 and 24 was associated with survival. • A total of 46.7% of patients with SD at week 12 achieved a best overall response of complete response (CR)/partial response (PR). • Patients with SD and then CR/PR had similar survival to those with maintained PR. • Patients with SD/PR at week 12 followed by progression had poor survival outcomes.
The concepts of rechallenge and retreatment with immune checkpoint blockade in melanoma patients
Highlights • Retreatment, rechallenge and escalation are options for patients who relapse. • Relapse during/after adjuvant PD-1 responds to targeted and ipilimumab-based therapy. • Rechallenge with PD-1 inhibitor in advanced setting can achieve disease control. • Escalation should be preferred to PD-1 inhibitor monotherapy rechallenge.
The role of mRNA translational control in tumor immune escape and immunotherapy resistance
Review Michaël Cerezo, Caroline Robert, Lunxu Liu and Shensi Shen DOI: 10.1158/0008-5472.CAN-21-1466 Abstract Tremendous advances have been made in cancer immunotherapy over the last decade. Among the different steps of gene expression, translation of mRNA is emerging as an essential player in both cancer and immunity. Changes in mRNA translation are both rapid and adaptive, and translational reprogramming is known to be necessary for sustaining cancer cell proliferation.

Immunotherapy discontinuation - how, and when? Data from melanoma as a paradigm
Abstract The optimal duration of therapy in patients receiving immune-checkpoint inhibitors (ICIs) is a new but crucial question that has arisen owing to the observation of durable remissions in >85% of patients with metastatic melanoma who stop receiving an anti-PD-1 antibody after a complete response (CR). Long-term treatment-free remissions have also been seen, albeit much less frequently, in patients receiving ICIs for other forms of cancer who have a CR.
One or Two Immune Checkpoint Inhibitors?
The combination of ipilimumab and nivolumab was evaluated versus standard treatments in melanoma and lung cancer. Analysis of these trials highlights the differences in outcomes and potential predictive biomarkers across tumor types as well as the multiple remaining questions concerning the optimal use of immune checkpoint inhibitors.
