State records reveal a serious workplace accident at the Nippon Dynawave pulp and paper mill in Longview involving hot pulp spilling from a faulty valve, creating a sinkhole that partially submerged an employee. The March incident at the mill does not appear to be related to the tank collapse that killed 11 workers more than two months later, according to documents released by the Washington Department of Labor and Industries.
