We thank Katrin Milger and colleagues for their thoughtful comments on our Series paper on anti-cytokine biologics for severe asthma.1 We agree that blood eosinophils should not be used in isolation and that multicomponent phenotyping—including age of onset, allergic status, biomarkers, and comorbidities—can help identify patients more likely to benefit from biologic therapy. However, the strongest predictor of response remains the baseline level of type 2 inflammation.
