Methods NSCLC cohort: EDTA plasma samples were collected from patients with advanced-stage NSCLC with consent as part of the multicenter PROPHETIC study (NCT04056247). All sites received approval from the local institutional review board to perform the study before commencing (online supplemental table S1). Sample collection was previously described.10 11 The patients received either ICI monotherapy (nivolumab, pembrolizumab, atezolizumab) or a combination therapy of ICI with chemotherapy.

Discussion Current biomarkers for predicting response to ICI therapy are mostly related to the tumor and immune cells within the tumor microenvironment. Such biomarkers include PD-L1 status of both tumor cells and tumor-associated immune cells, TMB, microsatellite instability and tumor immune cell infiltration.35 As such, these biomarkers require tumor biopsies, posing significant challenges. First, biopsies are not always available especially in different lesions of a metastatic disease.
