Biallelic variants in RNU2-2 cause a remarkably frequent developmental and epileptic encephalopathy
Abstract Neurodevelopmental disorders (NDDs) affect 2–4% of the population, are predominantly genetic and remain unsolved in ~50% of individuals. We show that rare biallelic variants in RNU2-2 are enriched and over-transmitted in individuals with unresolved NDDs. We define a recessive RNU2-2 syndrome, delineate its unique genetic architecture and show that it manifests clinically as a severe developmental and epileptic encephalopathy.

Hypoglycaemia impacts the lives of people living with diabetes, but there are no robust qualitative interview studies detailing in-depth and holistically the quality of life impacts of hypoglycaemia on people living with type 1 and type 2 diabetes. This study generates evidence for a novel and comprehensive 38-item conceptual quality of life framework for understanding the person-centred impacts of hypoglycaemia, including physical, psychological and social aspects.

MiniMed 780G system performance in older users with type 1 diabetes: Real‐world evidence and the case for stricter glycaemic targets
1 INTRODUCTION Life expectancy has steadily improved over time, including for people with type 1 diabetes, resulting in a growing number of older individuals living with this condition. Yang et al.1 recently estimated that, on a global level, there were 3.7 million people aged ≥65 years living with type 1 diabetes in 2019, including >0.9 million aged >80 years, compared with just 1.3 million aged ≥65 years in 1990.
Clinical perspectives on the frequency of hypoglycemia in treat-to-target randomized controlled trials comparing basal insulin analogs in type 2 diabetes: a narrative review
Introduction As recommended by the American Diabetes Association (ADA) and European Association for the Study of Diabetes (EASD) 2022 consensus on the management of hyperglycemia,1 basal insulin has been replaced by injectable incretin-related therapy as the first-line injectable therapy for type 2 diabetes (T2D) insufficiently controlled by combinations of oral glucose-lowering agents.
Diabetes prevention at scale: Narrative review of findings and lessons from the DIPLOMA evaluation of the NHS Diabetes Prevention Programme in England
1 INTRODUCTION Type 2 diabetes is a major clinical challenge.1 The World Health Organisation estimates that approximately 422 million people have diabetes worldwide and 1.5 million deaths are directly attributed to diabetes each year.2 Diabetes UK recently estimated that over five million people have diabetes in the UK3 and over two million people have been diagnosed with non-diabetic hyperglycaemia in England.4 The NHS Diabetes Prevention Programme (NHS DPP) is a national, publicly funded...
A parallel randomised controlled trial of the Hypoglycaemia Awareness Restoration Programme for adults with type 1 diabetes and problematic hypoglycaemia despite optimised self-care (HARPdoc) | Nature Communications
Abstract Impaired awareness of hypoglycaemia (IAH) is a major risk for severe hypoglycaemia in insulin treatment of type 1 diabetes (T1D).
Insulin, the patient, and the health professional
Before 1921 the diagnosis of diabetes was a death sentence, with an average life expectancy of less than 2 years in young people, who were only kept alive by a starvation regimen that reduced them to walking skeletons. 1 Tattersall RB The pissing evil: a comprehensive history of diabetes mellitus. Impact of kidney function on the safety and efficacy of insulin degludec versus insulin glargine U300 in people with type 2 diabetes: A post hoc analysis of the CONCLUDE trial
Chronic kidney disease (CKD) is a common complication of type 2 diabetes (T2D) that, in insulin-treated people can result in decreased insulin clearance and increased risk of hypoglycaemia.1 Severe hypoglycaemia is associated with increased morbidity and mortality, particularly in relation to cardiovascular disease (CVD).2, 3 CKD is associated with an increased risk of hypoglycaemic events in people with or without diabetes,1 and also independently increases the risk of CVD and death.4...

Evaluating glucose‐lowering treatment in older people with diabetes: Lessons from the IMPERIUM trial
Abstract Understanding the benefits and risks of treatments to be used by older individuals (≥65 years old) is critical for informed therapeutic decisions. Glucose‐lowering therapy for older patients with diabetes should be tailored to suit their clinical condition, comorbidities and impaired functional status, including varying degrees of frailty.

Redefining Hypoglycemia in Clinical Trials: Validation of Definitions Recently Adopted by the American Diabetes Association/European Association for the Study of Diabetes
Pathophysiology/Complications Abstract OBJECTIVE To determine if the International Hypoglycaemia Study Group (IHSG) level 2 low glucose definition can identify clinically relevant hypoglycemia in clinical trials and offer value as an end point for future trials.
Robert Tattersall, a Diabetes Physician Ahead of His Time
Profiles in Progress The clinical and research landscape in the U.K. was dominated in the 1970s and 1980s by clinician scientists. Many worked as full-time employees of the National Health Service rather than universities. During a period when health services were under less pressure than today, they were able to find sufficient time to design clinical studies to answer important research questions that occurred to them while treating their patients.

Response to Comment on Novodvorsky et al. Diurnal Differences in Risk of Cardiac Arrhythmias During Spontaneous Hypoglycemia in Young People With Type 1 Diabetes. Diabetes Care 2017;40:655-662 | Diabetes Care
We welcome the opportunity to respond to the comments from Home and Lachin (1) on our article (2). One of us (M.C.) has independently reexamined the nighttime bradycardia data using the negative binomial generalized estimated equations approach (SPSS version 22; IBM, Chicago, IL) described in our article. With autocorrelated error terms, he could approximately replicate our results (incident rate ratio [IRR] 5.7 [95% CI 5.0, 6.5], P < 0.001 vs.

High-sensitivity C-reactive Protein, Low-Density Lipoprotein Cholesterol, and Cardiovascular Outcomes in Patients with Type 2 Diabetes in the EXAMINE (Examination of Cardiovascular Outcomes With Alogliptin Versus Standard of Care) Trial - Hwang - 2017 - D
We sought to assess the risk of major adverse cardiovascular events (MACE) by utilizing high-sensitivity C-reactive protein (hsCRP) level and low-density lipoprotein cholesterol (LDL-C) in patients with type 2 diabetes and recent acute coronary syndrome. Study participants enrolled in the EXAMINE trial, were stratified by baseline hsCRP levels (3?mg/l) and were also sub-divided into 4 groups according to baseline hsCRP (?3 or >3?mg/l) and achieved LDL-C (<70 or ?70?mg/dl) levels.

Relationship of glycated haemoglobin and reported hypoglycaemia to cardiovascular outcomes in patients with type 2 diabetes and recent acute coronary syndromes: the EXAMINE trial - Heller - 2017 - Diabetes, Obesity and Metabolism - Wiley Online Library
To investigate relationships between glycated haemoglobin (HbA1c) and reported hypoglycaemia and risk of major adverse cardiovascular (CV) events (MACE). The EXAMINE trial randomized 5380 patients in 49 countries with type 2 diabetes (T2DM) and a recent acute coronary syndrome (ACS) to double-blind treatment with alogliptin or placebo in addition to standard of care. We used Cox proportional hazards models to analyze relationships among MACE, HbA1c levels and hypoglycaemia events.
