Interpreting the GRACE 3.0 ITE model: from predictive performance to clinical decision utility
Authors’ reply We appreciate Dr Zhang’s comments regarding our Article1 published in The Lancet Digital Health. Decision curve analysis based on mortality risk assumes that the threshold probability of mortality at which a patient or physician opts for the potential benefit of early invasive treatment (EIT) rather than delayed invasive management is informative of how they weigh the harms of a false prediction.
A gut microbiome-kidney-heart axis predictive of future cardiovascular diseases - Nature Communications
|
Abstract Cardiovascular diseases (CVD) remain a major global health challenge. Early markers of disease initiation and progression are urgently needed. We, and others, have previously shown changes in the gut microbiome in association with metabolic and CVD. Here, we demonstrate that gut microbiome-related changes can be detected in association with subclinical variations in heart and kidney function.

Subgrouping patients with ischemic heart disease by means of the Markov cluster algorithm - Communications Medicine
Abstract Ischemic heart disease (IHD) is heterogeneous with respect to onset, burden of symptoms, and disease progression. We hypothesized that unsupervised clustering analysis could facilitate identification of distinct and clinically relevant multimorbidity clusters. We included IHD patients who underwent coronary angiography (CAG) or coronary computed tomography angiography (CCTA) between 2004 and 2016 and used the earliest procedure as the index date.

Comparing Glucagon‐like peptide‐1 receptor agonists versus metformin in drug‐naive patients: A nationwide cohort study
1 INTRODUCTION In patients with type 2 diabetes with overweight and no cardiovascular disease or moderate risk of cardiovascular disease, metformin is the recommended initial treatment (class IIa recommendation).1 The Food and Drug Administration (FDA) did not approve glucagon-like peptide-1 receptor agonists (GLP-1 RA) (semaglutide) tablets for drug-naive type 2 diabetes until January 2023.2 To our knowledge, these recommendations were not based on any head-to-head randomized clinical trials...
Electronic nudges for sustained influenza vaccination uptake in older adults: the nationwide randomized NUDGE-FLU-2 trial - Nature Medicine
Abstract Digital letter interventions have proven effective in increasing influenza vaccination rates. In this trial, we sought to further refine these strategies and investigated whether the effectiveness of the strategies could be sustained across consecutive influenza seasons. We enrolled all eligible Danish citizens 65 years of age or older in a nationwide registry-based randomized implementation trial during the 2023–2024 influenza season.
Balcinrenone plus dapagliflozin in patients with heart failure and chronic kidney disease: Results from the phase 2b MIRACLE trial
Introduction In patients with heart failure (HF), concomitant renal dysfunction is common and negatively affects prognosis and complicates HF treatment.1 Although mineralocorticoid receptor (MR) antagonists are a foundational therapy for HF with reduced ejection fraction, they are substantially underused in patients with chronic kidney disease (CKD)2 and are not recommended in those with an estimated glomerular filtration rate (eGFR) <30 ml/min/1.73 m2.3, 4 Balcinrenone (previously AZD9977)...

Pre-eclampsia in a first pregnancy and subsequent pregnancy outcomes: a nationwide cohort study
Abstract Background For women whose first pregnancy was complicated by pre-eclampsia (PE), particularly if severe and requiring early birth, the risk of recurrence and maternal and neonatal outcomes at subsequent birth are important considerations. Methods In this observational cohort study, all primiparous women who gave birth in Denmark between 1997 and 2016 were identified using nationwide registries.
Evidence of a causal and modifiable relationship between kidney function and circulating trimethylamine N-oxide - Nature Communications
|
Abstract The host-microbiota co-metabolite trimethylamine N-oxide (TMAO) is linked to increased cardiovascular risk but how its circulating levels are regulated remains unclear. We applied “explainable” machine learning, univariate, multivariate and mediation analyses of fasting plasma TMAO concentration and a multitude of phenotypes in 1,741 adult Europeans of the MetaCardis study.

Influenza Vaccination Is Associated With Reduced Cardiovascular Mortality in Adults With Diabetes: A Nationwide Cohort Study
Cardiovascular and Metabolic Risk Abstract OBJECTIVE Recent influenza infection is associated with an increased risk of atherothrombotic events, including acute myocardial infarction (AMI) and stroke. Little is known about the association between influenza vaccination and cardiovascular outcomes in patients with diabetes. RESEARCH DESIGN AND METHODS We used nationwide register data to identify patients with diabetes in Denmark during nine consecutive influenza seasons in the period 2007–2016.
Prevalence and incidence of various Cancer subtypes in patients with heart failure vs matched controls
Highlights • Whether heart failure patients have an increased cancer risk is not well-known. • We observed that patients with heart failure had increased unadjusted cancer risk. • Adjusted for multiple factors, the risks were almost completely attenuated. Abstract Background Patients with heart failure (HF) may be at increased risks of cancer, but the magnitude of risk for various cancer subtypes is insufficiently investigated.
Infarct size following loading with Ticagrelor/Prasugrel versus Clopidogrel in ST-segment elevation myocardial infarction
Highlights • Newer P2Y 12 antagonists are superior to Clopidogrel in terms of clinical outcomes in patients with acute coronary syndromes. • The mechanism of action mediating this difference is unclear. • A plausible mechanistic explanation could involve a decrease in final infarct size.

Warfarin is a cornerstone for the prevention of thromboembolism in atrial fibrillation (AF), and several efforts have been taken to increase its usage and safety, including risk stratification schemes. Our aim was to investigate the temporal trends in initiation of warfarin and its effects on incidence of bleeding and thromboembolism in patients with new-onset atrial fibrillation 1996–2011.
Sacubitril/valsartan reduces serum uric acid concentration, an independent predictor of adverse outcomes in PARADIGM‐HF
Aims Elevated serum uric acid concentration (SUA) has been associated with an increased risk of cardiovascular disease, but this may be due to unmeasured confounders. We examined the association between SUA and outcomes as well as the effect of sacubitril/valsartan on SUA in patients with heart failure with reduced ejection fraction (HFrEF) in PARADIGM‐HF.
Contribution of cardiac and extra‐cardiac disease burden to risk of cardiovascular outcomes varies by ejection fraction in heart failure
Patients with heart failure (HF) often have multiple co‐morbidities that contribute to the risk of adverse cardiovascular (CV) and non‐CV outcomes. We assessed the relative contribution of cardiac and extra‐cardiac disease burden and demographic factors to CV outcomes in HF patients with reduced (HFrEF) or preserved (HFpEF) left ventricular ejection fraction (LVEF). We utilized data from the CHARM trial, which enrolled HF patients across the ejection fraction spectrum.

Between 40 and 50% of patients who present with acute ST-elevation myocardial infarction (STEMI) have multivessel coronary artery disease.1,2 Joint guidelines from the American College of Cardiology, the American Heart Association, and the European Society of Cardiology recommend treatment of the culprit vessel only, but data from a few trials of intermediate size favor complete revascularization, mainly on the basis of reductions in the need for repeat revascularizations.3-7 The severity of...
Infective endocarditis and risk of death after cardiac implantable electronic device implantation: a nationwide cohort study
Aims To determine the incidence, risk factors, and mortality of infective endocarditis (IE) following implantation of a first-time, permanent, cardiac implantable electronic device (CIED). Methods and results From Danish nationwide administrative registers (beginning in 1996), we identified all de-novo permanent pacemakers (PMs) and implantable cardioverter defibrillators (ICDs) together with the occurrence of post-implantation IE-events in the period from 2000-2012.
Defibrillator Implantation in Patients with Nonischemic Systolic Heart Failure
Given the limited evidence of a benefit from the implantation of an ICD in patients with chronic nonischemic heart failure, we conducted a randomized trial in which patients whose condition was stable and who had chronic symptomatic heart failure, reduced ejection fraction, and increased levels of natriuretic peptide with or without a need for CRT were randomly assigned either to receive or not to receive an ICD.
Short-term mortality risk of serum potassium levels in hypertension: a retrospective analysis of nationwide registry data
Aims Diuretics and renin-angiotensin-aldosterone system inhibitors are central in the treatment of hypertension, but may cause serum potassium abnormalities. We examined mortality in relation to serum potassium in hypertensive patients. Methods and results From Danish National Registries, we identified 44 799 hypertensive patients, aged 30 years or older, who had a serum potassium measurement within 90 days from diagnosis between 1995 and 2012.
