Research priorities to move ctDNA assays into clinical decision making in early-stage breast cancer
Abstract Circulating tumor DNA (ctDNA)-based Minimal Residual Disease (MRD) assays are increasingly utilised in early-stage breast cancer; but evidence for decision-making remains incomplete.
Importance The treatment of older adults with chemotherapy for early-stage breast cancer involves a complex decision-making process that weighs the competing risks of disease recurrence against the potential harms of treatment.
Right-sizing treatment for HER2-positive breast cancer: evolving algorithms across the disease continuum
Abstract Human epidermal growth factor receptor 2 (HER2)-positive breast cancer has undergone one of the most profound therapeutic transformations in oncology. Once associated with poor outcomes, it is now a highly curable disease in the early-stage setting, and a substantial proportion of patients with metastatic disease survive beyond 5 years.
Clinical responses to trastuzumab deruxtecan in molecularly defined ependymoma
Abstract Despite evidence that antibody drug conjugates (ADCs) are active in brain metastases, their role in primary central nervous system (CNS) tumors remains unclear. We report two adults with molecularly defined ependymoma – a MYCN-amplified spinal tumor with intracranial dissemination and a supratentorial ZFTA-RELA-fused tumor with multiple recurrences – who demonstrated radiographic response and/or durable clinical and metabolic stability with trastuzumab deruxtecan (T-DXd).
Circulating Tumor DNA in Early Breast Cancer : A Review
Abstract Importance Circulating tumor DNA (ctDNA) has emerged as a noninvasive biomarker with the potential to detect minimal residual disease (MRD), monitor treatment response, and identify recurrence (eg, molecular relapse) earlier than conventional clinical or imaging approaches. Although ctDNA-based MRD assays have demonstrated prognostic value in early breast cancer, their optimal clinical utility remains uncertain.
Carboplatin with or without nivolumab in metastatic triple-negative breast cancer: a randomized phase II trial
|
Abstract This multi-institutional randomized phase II clinical trial (NCT03414684) investigated the efficacy and safety of carboplatin plus nivolumab compared to carboplatin in metastatic triple-negative breast cancer (mTNBC). The primary endpoint was progression-free survival (PFS) in a modified intention-to-treat (mITT) population of patients with chemotherapy-naïve (i.e., first-line) mTNBC.

Quantitative HER2 tissue and plasma profiling predicts the activity of trastuzumab deruxtecan for breast cancer
|
Abstract Trastuzumab deruxtecan (T-DXd) is commonly used for treating metastatic breast cancer (MBC); however, traditional HER2 immunohistochemistry has largely failed to predict T-DXd activity. We reviewed survival outcomes and tested the reliability of multiple HER2 quantitative assays in predicting T-DXd’s performance among 191 patients with MBC.

Tumor genomics in patients younger than 40 years of age with metastatic breast cancer - npj Precision Oncology
|
Abstract Unique disease characteristics of younger patients warrants investigation of tumor genomics in young-onset metastatic breast cancer (MBC). Targeted DNA sequencing was completed for tumors of MBC patients diagnosed between 2009-2020. Multivariable logistic regression tested associations between single nucleotide variants (SNVs) and copy number variants and age at MBC diagnosis. Multivariable Cox regression estimated hazard ratios for overall survival (OS) by somatic alterations.

Androgen receptor expression and immune characteristics of HER2-low metastatic triple-negative breast cancer - npj Breast Cancer
|
Abstract HER2-low expression is associated with hormone receptor (HR) expression in HR-positive breast cancer. We aimed to evaluate its association with androgen receptor (AR) among 196 patients with metastatic triple-negative breast cancer (mTNBC). Central determination of AR showed significant enrichment in HER2-low compared with HER2-0 mTNBC (mean: 33.7% vs. 21.4%, p = 0.038), whereas no significant immunological differences were observed.

VIDEO: Further research needed on de-escalating, curative intent therapies in breast cancer
Editor’s note: This is an automatically generated transcript, which has been slightly edited for clarity. Please notify editor@healio.com if there are concerns regarding accuracy of the transcription. So, I think for early stage HER2-positive disease, we have seminal deescalation study in stage one patients, the APT trial which looked at paclitaxel (Bristol-Myers Squibb) with trastuzumab (Herceptin, Genentech), but that's still 12 weeks of paclitaxel that can cause neuropathy of trastuzumab.
VIDEO: Access to latest treatments, supportive care key in breast cancer
Editor’s note: This is an automatically generated transcript, which has been slightly edited for clarity. Please notify editor@healio.com All of these drugs are expensive. They require some amount of oncology expertise. As new drugs and new regimens come to the fore, we have to remember that most patients receive care from the general oncology community.
VIDEO: Accessibility, delays in diagnosis remain challenges in breast cancer
Editor’s note: This is an automatically generated transcript, which has been slightly edited for clarity. Please notify editor@healio.com Hello, my name is Dr. Nancy Lin. I'm a professor of medicine at Harvard Medical School and medical oncologist at Dana-Farber Cancer Institute in Boston, Massachusetts. So, I think for early-stage breast cancer, the challenge more has to do with accessibility.

Editor’s note: This is an automatically generated transcript, which has been slightly edited for clarity. Please notify editor@healio.com if there are concerns regarding accuracy of the transcription. Well, I think the major risk factor for metastatic breast cancer really is having had a diagnosis of early-stage breast cancer and the potential for relapse.
VIDEO: Questions remain as treatment improves in HER2-positive breast cancer
Editor’s note: This is an automatically generated transcript, which has been slightly edited for clarity. Please notify editor@healio.com if there are concerns regarding accuracy of the transcription. There have been a lot of developments in both early stage and metastatic treatment algorithms for patients with HER2-Positive disease. So in the early stage setting we saw results of two really important trials.

A multi-modal single-cell and spatial expression map of metastatic breast cancer biopsies across clinicopathological features - Nature Medicine
Abstract Although metastatic disease is the leading cause of cancer-related deaths, its tumor microenvironment remains poorly characterized due to technical and biospecimen limitations. In this study, we assembled a multi-modal spatial and cellular map of 67 tumor biopsies from 60 patients with metastatic breast cancer across diverse clinicopathological features and nine anatomic sites with detailed clinical annotations.
Osteopontin is a therapeutic target that drives breast cancer recurrence - Nature Communications
Abstract Recurrent breast cancers often develop resistance to standard-of-care therapies. Identifying targetable factors contributing to cancer recurrence remains the rate-limiting step in improving long-term outcomes. In this study, we identify tumor cell-derived osteopontin as an autocrine and paracrine driver of tumor recurrence. Osteopontin promotes tumor cell proliferation, recruits macrophages, and synergizes with IL-4 to further polarize them into a pro-tumorigenic state.
